8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Technical Assistance:
Discussed getting permission from the parents to take the children on walks.

Comments:
An in-person annual subsidy inspection was conducted on 3/09/2023 from 9:40am to 11:00am. There were (4) children in care with the vendor. The inspector reviewed (4) children?s records and (6) adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Violations:
Standard #: 8VAC20-790-180-A
Description: Based on observation, the caregiver records and children's information was not kept confidential.
Evidence:
1. The supplemental page dated 2/27/2020 that is considered confidential was posted on the bulletin board for others to view.'

Plan of Correction: I took it down.

Standard #: 8VAC20-790-190-B
Description: Based on review of children's Records, the child information does not include all required information.
Evidence:
1. Child #1's file was missing one parent's work phone number and a written agreement between the parent and vendor.
2. Child #2's file was missing both parent's work phone number and one parent's place of employment and address, a written agreement between the parent and vendor and the immunization record.
3. Child #3's file was missing one parent's home address, work address and work phone number and a written agreement between and the parent and vendor.
4. Child #4's file was missing one parent's work phone number and the written agreement between the parent and vendor.

Plan of Correction: I promise to have the information for the records.

Standard #: 8VAC20-790-230-A
Description: Based on record review, the vendor did not obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations before the child attended the family day home.
Evidence:
1. Child #2 (Start Date 2/23/2023) was in attendance during the inspection and the vendor did not have the child's immunization record.

Plan of Correction: I promise to send you the vaccination record before 3/18/2023.

Standard #: 8VAC20-790-360-G
Description: Based on observation, the vendor did not provide cots, rest mats or beds for children 12 months of age or older.
Evidence:
1. The vendor had one cot, but had 4 children (Approximate ages 16 months, 18 months, 3 years and 5 years) in attendance. She stated the children sleep in the baby swings and one sleeps on the cot.

Plan of Correction: I already bought 4 rest mats and 4 sheets for the cribs.

Standard #: 8VAC20-790-410-B-2
Description: Based on observation, the provider did not have a working battery operated radio.
Evidence:
1. The provider had a radio, but did not have batteries to put in it to make it work.

Plan of Correction: I already bought batteries for the radio.

Standard #: 8VAC20-790-430-B
Description: Based on review, the vendor did not maintain a record of the dates of the practice drills for one year.
Evidence:
1. The vendor had record of drills in January 2023 and February 2023, but did not have record of 10 months of the drills in 2022.

Plan of Correction: I also promise to have the exercise record and the information for the records.
