A scheduled new facility/modification inspection was conducted on 5/12/2023 from 11:00 am to 12:15 pm. There were no children in care. The Licensing Inspector reviewed Board Officer backgrounds, three staff records, and other program records. The licensee is moving back to their original location.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Note: the inspection was amended on 5/23/2023 after discussion with LA.

Standard #: 22.1-289.036-A Description: Based on document review, the center failed to ensure that an applicant for licensure had a complete background check every five years.
EVIDENCE:
The most recent fingerprint background check for staff 1 was dated 3/5/2018.Plan of Correction: Staff 1 has schedule the fingerprint background check and will forward results to the LI when they have been obtained.
