Comments:
A monitoring inspection was initiated on 7/5/2023 and concluded on 7/5/2023. There were 59 children directly supervised by 9 staff members. Children were observed during snack time. The physical plant, 3 child records, 3 staff records, medications, evacuation drills, emergency supplies and attendance were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-2
Description: Based on a review of staff records, fingerprint results were not obtained prior to first day of employment.
Evidence: The following staff do not have documentation of fingerprint results in their staff file and were observed this day working with children;
Staff #2 and staff #3, both hired in 6/2023.

Plan of Correction: Staff #2 and #3 will not be allowed back to work until Fingerprint results have been obtained. The results of the staff's fingerprints will be emailed to the Inspector.

Description: Based on a review of staff records, staff have not submitted documentation of a negative tuberculosis screening prior to hire.
Evidence: Staff #1, date of hire 6/2023 does not have documentation of a TB screening.

Plan of Correction: Staff will obtain TB screening and director will email the results to the Inspector.

Description: Based on a review of staff records, documentation of at least 2 references has not been obtained for each staff member

Plan of Correction: Date of employment will be noted on each staff members file.

Description: Based on a review of staff records, staff have not completed orientation training prior to working with children.
Evidence: Staff #1, #2, and #3 all hired in 6/2023 were observed working with children this day without having completed orientation training.

Plan of Correction: Orientation will be completed with each staff member and proof of that training for the 3 staff noted will be emailed to the Inspector.

Description: Based on a review of staff records, staff have not completed the required Recognizing child Abuse and Neglect training prior to working alone with children.
Evidence: Staff #2 and #3, both hired in 6/2023 does not have documentation that they completed Recognizing Child Abuse and Neglect training.

Plan of Correction: Staff will take the required training and the certificates of completion will be emailed to the Inspector.

Standard #: 8VAC20-780-245-L
Description: Based on a review of staff records, there is not a staff on duty at all time that has been trained in Daily Health Observation.
Evidence: One staff who works 7:30am-4:00pmhas daily health observation. There is no staff with Daily Health Observation training from 4:00 pm until closing at 6:00 pm.

Plan of Correction: Staff will take the Daily Health Observation training so that there is one staff at all hours of operation that has the training.
