Technical Assistance:

Transporting children on public school buses was discussed with the program administrator on 6/28/2023. Children's emergency medications was discussed with the program director. An environmental health inspection has been requested.

Comments:

A staff record review was conducted on 6/28/2023 and a monitoring inspection on site was conducted on 7/5/2023. There were 85 school aged children in care with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 15 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Staff record violations were discussed with the program administrator on 6/28/2023.

Standard #:	8VAC20-780-160-A-1
Description:	Based upon review of staff records, the facility has not ensured that each staff member submits the results of a tuberculosis screening at the time of employment and before coming into contact with children. Evidence: The records for staff 6 and 14, who were employed and began working on 6/26/2023, do not include documentation of tuberculosis screenings.
Plan of Correction:	The facility responded with the following: Bath staff 6 and 14 have received a cleared TB assessment.
Standard #:	8VAC20-780-160-A-2
Description:	Based upon review of staff records, the facility has not ensured that each staff member has submitted documentation of a tuberculosis screening that was documented within 30 days prior to employment. Evidence: The tuberculosis screening in the record provided for staff 2, hired on 6/26/2023, is dated 9/3/2022.
Plan of Correction:	The facility responded with the following: Staff will be better advised of the "not more than 30 days prior" requirement. Staff #2 to complete by 7/24 - staff 2 is currently out for a planned vacation.
Standard #:	8VAC20-780-70
Description:	Based upon review of records, the facility has not ensured that each staff record includes all required information. Evidence: 1. The records provided for staff 1, 2 and 15 do not include documentation that at least two references as to character and reputation as well as competency were checked before employment. 2. The records provided for staff 3, 4, 5, 7, 8. and 14 do not included the street addresses for the emergency contact persons.
Plan of Correction:	The facility responded with the following: We will ensure reference checks are completed / and recorded properly as a first step going forward As of 7/19/2023 we have obtained 2 positive references on staff 1, 2, and one on staff 15, with requests our for the 2nd - or an alternate Addresses have been recorded in the file and/or an all staff spreadsheet.
