Comments:
An unannounced monitoring inspection was conducted from 1:45 pm - 3:15 pm. There were 54 children in care being supervised by 6 staff. The children were observed participating in various cooperative group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-40-M
Description: Based on observation and interviews it was determined that the facility failed to maintain and make accessible to staff a current written list of all children's allergies, sensitivities, and dietary restrictions for children enrolled in the program. Evidence: Staff were not able to produce a written allergy list for all children enrolled with allergies, sensitivities, and dietary restrictions as required.

Description: Based on review of three staff records it was determined that not all records were complete. Evidence: 1.) The records for Staff A (start date: 5/1/23), Staff B (start date: 6/19/23), and Staff C (start date: 5/16/22) did not contain a telephone number of a person to be notified in an emergency. 2.) The records for Staff B (start date: 6/19/23) and Staff C (start date: 5/16/22) did not contain documentation to demonstrate the highest level of education completed as required by their staff position.

Description: Based on review of attendance records and interviews it was determined that the arrival and departure of each child was not documented. Evidence: 1.) The attendance record for group one reflected 26 children in care with 22 children present. 2.) The attendance record for group two reflected 14 children in care with 6 children present. 3.) The attendance record for group four reflected 18 children in care with 19 children present.
