Comments:
A renewal inspection was conducted today between 12:00pm and 1:30pm. There were 190 children present, with 37 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed playing soccer in the gym, playing in the moon bounce outside and lining up to go outside. A total of 5 child records and 11 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-160-C
Description: Based on review of 11 staff records, the center did not obtain at least every two years from the date of the first initial screening or testing obtain and submit the results of a follow-up tuberculosis screening for 2 staff members.
Evidence
Staff #1, date of hire 6/11/21, has an expired TB screening result in their file. Results in file are expired 6/18/2023.
Staff #2, date of hire 6/11/23, has an expired TB screening result in their file. Results in file expired 9/27/22.

Plan of Correction: I will follow up again with the staff to make sure they get the TB screening completed.

Standard #: 8VAC20-780-60-C
Description: Based on review of 5 children's records, the center kept an electronic copy of the proof of identity, without documenting the date and who saw the original proof of identity.
Evidence
For child #1, 2,3,4 and 5 there was an electronic copy of their proof of identity, with no documentation of who saw the original document or the date it was observed.

Plan of Correction: We will begin using a new system to review the original proof of birth before a child starts. The person who reviews it will sign off that they saw it.

Standard #: 8VAC20-780-240-A
Description: Based on review of ten staff records, seven do not contain documentation of the required Virginia Department of Education-sponsored orientation course completed within 90 calendar days of employment.
Evidence Staff #3,4 and 5 have not completed the required Virginia Department of Education sponsored orientation course.

Plan of Correction: Staff have been working on completing the training, They are almost completed. We will ensure they are completed.

Standard #: 8VAC20-780-450-A
Description: Based on observation cots and mats used by children during the designated rest period did not have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
Evidences
No linens or blankets are being used by the children.

Plan of Correction: We did not know they need to use a blanket, we will notify the parents to send in a blanket with their children.
