Comments:
A monitoring inspection was initiated on 7/27/2023 and concluded on 7/27/2023. Upon the Inspectors arrival, there were 21 children directly supervised by 3 staff members. Children were observed arriving to the summer camp playing games in the gym. The physical plant, 3 child records, 4 staff records, medications, and attendance were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-2
Description: Based on a review of staff records, staff have not submitted to fingerprints prior to the fist day of employment.
Evidence: Staff #2, date of employment 6/1/2022 does not have documentation of fingerprint results. The staff was observed caring for children this date.

Plan of Correction: The staff will not report to work until the fingerprint results are filed in the staff's record. All staff will have fingerprint results filed in the staff record maintained at the summer camp location.

Standard #: 22.1-289.035-B-4
Description: Based on a review of staff records, the child day center did not obtain a copy of the results of the criminal history record, sex offender registry check and a search of the child abuse and neglect registry from a staff who has resided in another state in the past 5 years.
Evidence: staff # 3, date of employment 6/5/2023 indicated they have resided in another state in the past 5 years, there is no documentation of the out of state background checks. The staff was observed caring for children this date.

Plan of Correction: Management will provide results of staff's out of state background checks, the results will be filed in the staff record maintained at the summer camp.

Standard #: 8VAC20-770-60-B
Description: Based on a review of staff re cords, staff were employed without the person's completed sworn statement.
Evidence: Staff #1, date of employment 6/12/2023 does not have documentation of completing the sworn statement prior to employment. The staff was observed caring for children this date.

Plan of Correction: Staff will complete the sworn statement and file in the staff record.

Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records, staff do not have results from the central registry finding within 30 days of employment.
Evidence: Staff #1, date of employment 6/12/2023 does not have documentation in the record of the central registry finding. Staff was observed caring for children this date.
Staff # 3, date of employment 6/5/2023 does not have documentation in the record of the central registry finding. Staff was observed caring for children this date.
Staff #4, date of employment 6/19/2023 does not have documentation in the record of the central registry finding. Staff was observed caring for children this date.

Plan of Correction: Management will follow up on staff's missing central registry results. All staff will have central registry results filed in the staff record maintained at the summer camp location.

Description: Based on a review of children records, the center has not obtained documentation of each child's immunization record before the child can attend the center.
Evidence: Child #1, date of enrollment 7/3/2023 does not have documentation of the immunization record. The child was in care this date.

Plan of Correction: Management will obtain a copy of the child's immunization record and place it in the file.

Standard #: 8VAC20-780-240-C
Description: Based on a review of staff records, staff have not completed the required Recognizing Child Abuse and Neglect training prior to working with children.
Evidence: Staff # 2, hire date 6/1/2022 does not have documentation that they completed Recognizing Child Abuse and Neglect training. Staff was observed caring for children this date.
Staff # 3, hire date 6/2023 does not have documentation that they completed Recognizing Child Abuse and Neglect training. Staff was observed caring for children this date. (Repeat violation)

Plan of Correction: Staff will take the Child Abuse training and file the certificate in the staff file.
