The poison ivy that is just beginning to creep through the playground fencing was discussed with the program director.
- Comments:

A monitoring inspection was conducted on 7/26/2023. The inspector arrived at 10:00 AM and departed at 11:40 AM. There were 80 school aged children present with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Standard #: 22.1-289.035-B-3 Description: Based upon review of staff records and staff interview, the facility has not ensured that they have obtained a copy of the results of a search of the child protective services central registry search.
Evidence:
1. The records provided for staff 1, employed on 6/23/2017, staff 2, employed on 6/23/2023 and staff 4, employed on 6/23/2023 did not contain the results of a central registry search.
2. Staff 1 acknowledged that the records listed did not include the results of central registry searches.Plan of Correction: The facility responded with the following:
Central registry searches have been requested for the listed staff. requests will be researched as to the results printouts. If the results cannot be obtained, new requests will be submitted.
Standard #: 8VAC20-780-280-B Description: Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked places using a safe locking method that prevents access by children.
Evidence:
1. There were six containers of keyboard duster spray with warnings labels on a table on the far side of the art room.
2. Staff 1 acknowledged the unlocked cans of keyboard duster.Plan of Correction: The facility responded with the following:
All staff will be reminded to check their spaces for potentially hazardous substances to ensure that all are locked as the classrooms may be accessed by others during times when the program is not in session.
