Technical Assistance:
Model forms for written attendance and emergency drill documentation will be forwarded to the Vendor.

As a reminder, please be sure that house pets such as birds are not in areas accessible to children in care.

Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. Observed 1 child (1 point) directly supervised by thevendor/provider. Ratio and supervision were in compliance. Child in care was engaged with a ruler and enjoyed measuring objects around the home. Positive interaction was observed between the Vendor and the child. No identified food allergies and no medication needs at this time. The exit interview was conducted with thevendor/provider. Inspection was conducted between 11:20am and 12:30pm. Standard #: 8VAC20-790-230-A
Description: Based on child record review, documentation of immunizations were not obtained for one child.
Evidence:
Documentation that Child # 3 was adequately immunized was not found.

Plan of Correction: The Vendor will request the immunization record from the parent.

Standard #: 8VAC20-790-430-A-2
Description: Based on review of emergency drills, shelter in place procedures were not practiced.
Evidence:
Two shelter in place procedures were not practiced in 2022.

Plan of Correction: A shelter in place drill will be done in one week. The Vendor will do two shelter in place drills every year.

Standard #: 8VAC20-790-430-A-3
Description: Based on review of emergency procedures, lockdown procedures were not practiced annually.
Evidence:
An annual lockdown drill was not conducted in 2022.

Plan of Correction: A lock down drill will be done in one week. Vendor will do a lock down drill once every year.
