An unannounced subsidy health and safety inspection was conducted on 5/22/2023 beginning at 10:10am and ending at 11:00am. There were 4 children in attendance at the time of the inspection, 1 child was the son of the vendor. Required background searches, training certificates and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Evidence: There was no name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached for Child #2 and Child #3.Plan of Correction: Vendor will store emergency contact information for both children in the children's files.
