Regarding standard 8VAC20-780-80-A: The arrival and departure of each child in care as it occurs should be clearly documented so staff can articulate an maintain an accurate accountability of children in care at all times.

Comments:
An unannounced renewal study was conducted from 11:00 am - 12:45 pm. There were 55 children in care being supervised by 7 staff. The children were observed participating in swimming activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-40-M
Description: REPEAT VIOLATION
Based on interviews and documentation it was determined that center failed to maintain a list of children's allergies, sensitivities, and dietary restrictions that was dated and kept in each area where children are present. Evidence: A piece of paper was provided to the LI (licensing inspector) that was said to be the current allergy list. The list contained two names with a food written next to the children's names. The list was not labeled to indicate that it was an allergy list, and was not dated. The list is kept in a closet located in the gym. The LI observed the children at a swimming activity on the facilities' grounds and staff were not able to articulate to the LI which children have food allergies. A list of children with allergies, sensitivities, and dietary restrictions was not available with the group at the swimming activity.

Standard #: 8VAC20-780-340-A
Description: Based on observation and interviews it was determined that staff did not always ensure the care, protection, and guidance of children while in their care. Evidence: A large group of children were observed at a swimming activity at the location's onsite pool. Staff did not have access to the group's attendance record and could not articulate how many children where in their care. It took approximately ten minutes for staff to retrieve the attendance record from the second floor gym and to conduct a name to face assessment to determine how many children were in care.

Standard #: 8VAC20-780-510-E
Description: Based on review of two emergency medications it was determined that the center failed to follow their medication procedures as required. Long term prescription drug use and over-the-counter medication may be allowed with written authorization from the child's physician and parent. Evidence: The record for Child A did not contain parental or physician's authorization to administer an emergency medication.
