Technical Assistance:
Consultation was given on the following:
1. The vendor will need to complete 16.0 hours of annual training by the end of July 2023.
2. Model forms including allergy action plans were highlighted
3. Children must be seated when eating or drinking from a bottle to prevent choking hazards.

Comments:
An unannounced non-mandated subsidy inspection was conducted at the Local Ordinance Home (LOH). Two children were present with one caregiver for 3 points which is within ratios for supervision. Observed drop-off, food preparation, free play, physical space and records. Reviewed two children's records, the vendor and household members background checks, training requirements and emergency preparation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on vendor statement, a written list is not maintained of all children's allergies, sensitivities or food preferences.
Evidence:
The vendor stated that no list was maintained.

Plan of Correction: A list was made.

Standard #: 8VAC20-790-390-E
Description: Based on vendor statement and observation, the diapering surface was not non-absorbent.
Evidence:
The diaper pad was covered with a cloth cover and the vendor stated that the chil's diaper is changed on the carpet.

Plan of Correction: Diaper pad was moved and pad is now placed on floor and disinfecting.

Standard #: 8VAC20-790-400-B-2
Description: Based on observation, a prescription drug was not maintained in the original labeled container.
Evidence:
An emergency medication was stored loosely in a backpack and was not in the prescription box with identifying label from the pharmacy.

Plan of Correction: Medication was stored in box.

Standard #: 8VAC20-790-400-H
Description: Based on observation and vendor statement, a medication was not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The vendor stated that the emergency medication for Child #2 was kept in the child's backpack and not locked up.
2. Observed household medication on the kitchen counter behind the couch.

Plan of Correction: Medication was locked in cabinet with safety lock.

Standard #: 8VAC20-790-450-G
Description: Based on observation, a bottle of formula was heated using a prohibited method.
Evidence:
A bottle of formula was heated in a microwave.

Plan of Correction: Formula heated on stove.
