1. Reviewed background check requirements, as applicable to staff, volunteers, and contract employees.
- Comments:

9:50am-12:20pm Conducted an unannounced religious exempt program inspection. Observed 57 children + 11 direct-care staff + 1 parent volunteer. Ratios and supervision were in compliance. Children were observed in a variety of activities, to include: art (coloring, painting), blocks, play dough, creative expression, outdoor active play, and visiting with local firefighters who brought a fire truck to the school today. Staff were actively engaged with the children and activities. The facility was found to be very clean, safeguarded appropriately, and well supplied with educational toys and equipment for the children. The outdoor play space was found to be well maintained today. Artwork made by the children was displayed throughout the school. Staff training and health requirements were in compliance. Administrative recordkeeping was well maintained. Policies and procedures are implemented, as required. One background check non-compliance was found today.

Standard #: 63.2(17)-1720.1-B-3 Description: Based on records reviewed and interview conducted, it was determined that 1 of 7 staff records did not include an out-of-state CPS check. Evidence: Staff #7 indicated on the sworn disclosure statement, dated 9/5/18, that she lived in another state in the last 5 years. The Director said that a request for the out-of-state CPS check has not been submitted. Plan of Correction: The out-of-state CPS check will be submitted in the next 10-days, and a copy of the application sent to the inspector.
