An in-person annual subsidy inspection was conducted on 7/31/2023 from 11:00am to 11:30am. There were no children in care with the vendor. The inspector reviewed (2) children?s records and (5) adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Standard #: 8VAC20-790-190-B Description: Based on review of children's records, two files did not include all required information.
Evidence:
1. Child #1 and Child #2 (start date 7/10/2023 and end date 7/27/2023) were missing proof of identity and their immunization record.Plan of Correction: I will make sure I have in the future.
Standard #: 8VAC20-790-230-A Description: Based on review of children's records, the vendor did not obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
