A discussion was held with vendor on only having 4 children in care at one time. It is ok to have more children enrolled, but only 4 can be in attendance at one time. We also talked about ensuring the repeat violations are corrected.

A subsidy health and safety (SHSI) inspection was conducted today between 10:15am and 11:30am. There were no children present with 1 provider/vendor supervising. The physical plant, policies and procedures, emergency supplies and procedures, 4 children's records, 1 provider/vendor record and 4 household member records were reviewed. The exit interview was conducted with the provider/vendor. Areas of non-compliance are identified in this report. The last day of documentation of daily attendance that documented the arrival and departure of each child was March 9, 2023.Plan of Correction: I will begin to immediately document the arrival and departure of all children as it occurs.

The record for child #1 was missing the work phone number for both parent's.
The record for child #2 was missing the work address for one parent.Plan of Correction: I will get the missing addresses and phone numbers. I'll make sure to get all required information.
Standard #: 8VAC20-790-430-A-1 Description: Based on review of evacuation procedures, drills were not practiced as required.
Evidence
Evacuation procedures were not practiced at least monthly;
Shelter in place procedures were not practiced twice a year
Lockdown procedures were not practiced at least annually.Plan of Correction: I will document fire drills monthly, shelter in place drills two times a year and one lockdown drill a year.
