This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
An annual Subsidy Health and Safety Inspection (SHSI) was initiated on November 3, 2021 and concluded on November 5, 2021. The provider was contacted by telephone and a virtual inspection was conducted beginning at 6:10 p.m. There were no children in care during the inspection. The inspector reviewed compliance in the areas including general record keeping, children's information, caregiver records, home maintenance, ratio requirements, medication, special care and emergencies and nutrition. A total of two children's records, one staff record and three household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-665-190-B Description: Based on review of documentation, it was determined that the caregiver did not undergo a TB screening every two years as required. Evidence: The most recent TB screening was dated 2/8/2019. Plan of Correction: I have an appointment made for 11-12-2021. I will forward the results once completed.
