10:55am-1:15pm Conducted an unannounced religious exempt program inspection. Observed 39 children + 10 direct-care staff. Ratios and supervision were in compliance. Children were observed eating snack, having story time and circle time, and outdoor active play. Staff were actively engaged with the children and activities. The facility was found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment today. A multi-purpose room and outdoor playground/activity space was also inspected. Staff training requirements were met. Administrative and background check non-compliances found.

Standard #: 63.2(17)-1716-A-6 Description: Based on interview conducted, it was determined that required information has not been disclosed in writing to parents/guardians and the general public. Evidence: The Director stated that information physical facilities, enrollment capacity, health requirements for staff, and public liability insurance has not been put in writing. Plan of Correction: This information will be put on the school's website.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on records reviewed, it was determined that 1 of 6 staff records did not include an out-of-state CPS check. Evidence: According to the sworn disclosure statement signed by Staff #2 on 9/12/18, an out-of-state CPS check was needed. The Director said that it was an oversight and not requested. Plan of Correction: The out-of-state CPS application will be submitted in the next 10-days, by 6/1/19, and a copy sent to the inspector.
