Comments:
A Subsidy Health and Safety inspection was conducted on My 30, 2019 to review supplemental health and safety requirements. LI Anderson arrived & started inspection at 10:45 AM. There were forty-nine (49) children ages 4 months-10 years old present under the direct care of eight (8) staff members. The Center Director and the church Pastor/Administrator were also both present during the inspection. The Licensing Inspector reviewed ten (10) children & ten (10) staff records. Center does not administer medication. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Licensing Inspector conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Center Director on May 30, 2019. Inspection concluded at 4:45 PM. Standard #: 22VAC40-665-710-F-1
Description: Based on inspection & discussion with staff, the Vendor failed to ensure cribs are provided for children under12 months of age. EVIDENCE: Licensing Inspector observed a child older than 12 months of age occupying a crib during rest period in the toddler/1 year old classroom. Staff confirmed that the child was in fact older than 12 months old.

Plan of Correction: Children 12 months of age and older will use a cot or mat during nap time. Corrected during inspection

Standard #: 22VAC40-665-710-F-2
Description: Based on inspection, the Vendor failed to provide a cot/rest mat for children older than 12 months of age. EVIDENCE: In the 2 year old classroom, the Licensing Inspector observed a child resting on the floor with blanket. Child was not placed on a cot/mat.

Plan of Correction: Each child over 12 months old will be provided with a cot/mat during nap/rest period. Corrected during inspection

Standard #: 22VAC40-665-710-I
Description: Based on observations, the Vendor failed to ensure pillows and filled comforters are not used by children less than 12 months of age while sleeping/resting. EVIDENCE: In the infant classroom, Licensing Inspector observed infant sleeping/resting on a boppy-pillow in his designated crib.

Plan of Correction: Will discontinue using boppy-pillow in infant cribs. Corrected during inspection.

Standard #: 22VAC40-665-720-B
Description: Based on inspection, the Vendor failed to ensure that pillows are used they are assigned for individual use and covered with a pillow case. EVIDENCE: In the 2 year old classroom, the Licensing Inspector observed a child using a pillow without a pillow case.

Plan of Correction: A pillow case will be used when a pillow is used by children.

Standard #: 22VAC40-665-740-A-1
Description: Based on inspection, the Vendor failed to ensure children's hands are washed with soap and running water before and after eating meals or snacks. EVIDENCE: Staff did not wash the children's hands in the 2 year old classroom after lunch. Children were observed leaving the table after they finished eating and went to their individual mats/cots to rest.

Plan of Correction: Children will wash their hands with soap and running water before and after meals/snacks.

Standard #: 22VAC40-665-770-B-5
Description: Based on inspection, the Vendor failed to have continuity of operations procedures to ensure that essential functions are maintained during an emergency. EVIDENCE: The Center's Emergency Preparedness Plan did not include continuity of operations procedures.

Plan of Correction: Director will update the emergency preparedness plan to include procedures addressing continuity of operations
