63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted on 6/7/19 from 9:35 am until 12:35 pm to review the religious exempt requirements. At the time of the inspection 16 children were present with three staff. The sample size consisted of five children's records and four staff's records. Children and staff were observed during TV time, art activity, lunch, bathroom break, hand washing, nap, transitions and behavioral management. Standard #: 63.2(17)-1716-A Description: Based on a review of the parent handbook and interview, the facility failed to provide a written disclosure to parents and guardians of the children in the center the qualifications of the personnel. Evidence: 1. The parent handbook did not contain information regarding personnel qualifications. 2. The director stated they post this information but do not provide it in writing to parents. Plan of Correction: The parent handbook will be updated to include the personnel qualifications.

Evidence: 1. The parent handbook did not contain information regarding the physical facilities, enrollment capacity, health requirements for staff, and public liability insurance. 2. The director stated they post this information but do not provide it in writing to parents. Plan of Correction: The parent handbook will be updated to include this information.
Standard #: 63.2(17)-1716-B-3 Description: Based on interview, the facility failed to establish and implement a procedure in which daily simple health screening and exclusion of sick children is completed by a person trained to perform such screening. Evidence: The director stated they have a policy for exclusion but no staff has been trained in daily health screening. Plan of Correction: The director will complete training in daily simple health screening.
