Comments:
An unannounced Subsidy Health & Safety inspection was conducted today, June 6, 2019 to determine the center's compliance with Subsidy Health and Safety vendor requirements. The inspection was initiated at 2:15pm and concluded at approximately 5:20pm. No children were present during this inspection. All subsidy requirements were reviewed and compliance determined. Administrative records and all areas in the center used by children were inspected. See attached notice for violations found during this inspection. Violations:
Standard #: 22VAC40-665-530-4
Description: Based on the inspection of administrative records, the required first aid/cardiopulmonary resuscitation (CPR) certifications have expired. Evidence: During the inspection of two staff records, it was discovered staff #2 received certifications in first aid and CPR in March 2017. These certifications expired in March 2019 with no further updates in the record to review.

Plan of Correction: Per administrator: We are scheduled to take the first aid/CPR training course in 06/15/19.

Standard #: 22VAC40-665-540-B
Description: Based on the inspection of administrative records, the required tuberculosis test (TB) or screening update was not conducted.

Evidence:
During the inspection of two staff records, it was discovered staff #2 with a completed TB test dated 05/26/17 expired on 05/26/19 with no further updates in the record. Subsidy health & safety requirements stipulates for the center to obtain documentation that each staff has been evaluated by a health professional and a statement that the individual is believed to be free of communicable tuberculosis with documentation maintained in the individual?s record.

Plan of Correction: Per administrator: I will have the staff to get an updated TB test by next week.
