Comments:
An unannounced complaint inspection was conducted onJune 7, 2019 from 10:00 a.m. to 11:00 a.m. in response to a complaint that was received in the licensing office on May 31, 2019 regarding alleged noncompliance in the areas of supervision / ratios of adult to children and physical facilities. The information gathered during the investigation supports the allegations, so the complaint is found to be valid. Violations were cited in the areas of adult to child ratios/ staffing and physical facilities. The violations appear on the violation notice.

Complaint related: No
Description: Based on observation, the child day center did not have sufficient supervisory personnel present according to the following ratio of staff to children: One staff member to four children from zero to twenty-four months. Evidence: On June 7, 2019 at approximately 10:00 a.m. the inspector observed 10 children between 17 months and 2 years old in a classroom with 2 staff. Staff stated that these children were normally in two classrooms; four children under 2 years old in one room and one staff and 7 children who are two years old in another class with 2 staff. Staff stated that due to a teacher calling out that day, they combined in one room to assist each other.

Plan of Correction: The administrator stated that due to 2 staff calling out, their normal protocol which meets the required ratios of adults to children was altered. The administrator stated that she will remind classroom staff to be aware of the adult to child ratios for the ages of children in their group and the number of children present. When the maximum number of children are present and more children are expected, they will contact a member of administration to look at other classrooms where staff may be utilized or to allow children to visit other classrooms, as appropriate, to ensure that ratios of adults to children are met at all times.

Description: Based on observation, the center has not ensured that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment. Evidence: The inspector observed a group of children playing on 2 portable, plastic sliding boards with steps that were located on the wooden deck outside of the center. Staff stated that these are brought out when the playground is too wet to be utilized for outdoor play. There was no cushioning material under the slides. In addition, there were several large pieces of composite play equipment and a set of swings located on the outdoor play space. None of these pieces of equipment had cushioning material under and around them. A child could fall or jump from any of the outdoor play equipment and sustain an injury.

Plan of Correction: The administrator stated that they had already begun making plans to have mulch installed under and around the outdoor equipment. They planned to have it before summer. In addition, the administrator plans to place foam mats or other cushioning material under the small slides when they are being utilized by the children.
