Technical Assistance:
Model forms for medication and sick policies will be emailed to the Vendor as a resource.

Vendor discussed interest in becoming a licensed provider. Contact was made with Licensing staff during the inspection and the Vendor was enrolled in the next Phase II training in October.

Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. The Vendor was not home at the beginning of the inspection but was contacted by phone and arrived within 20 minutes. No children in attendance today except the Vendor's own children who are 14 years and 16 years old. A total of 3 children are currently enrolled. Walk thru of all rooms in the home was made. The home was found neat and organized. Discussed the routine of keeping the sliding glass doors to the balcony locked and off limits to children. Artwork completed by children was on display on the walls of the program space. The diaper changing routine was reviewed and the cleaning agents are located in an area that is secure. No use of cribs at this time for the children currently enrolled. This inspection occurred between 11:30am and 12:50pm. If you have any questions regarding today's inspection, please contact:

Description: Based on child record review, children's records were incomplete.
Evidence:
1. Child #1, 2 and 3's records did not include information about whether the Vendor administer medication.

Plan of Correction: I will use the model form that will be sent to me.
