8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Technical Assistance:
A discussion was held on organizing the children's records, keeping the physical plant free of items the children can trip on and paperwork organization.

Comments:
A subsidy health and safety (SHSI) inspection was conducted on today between 10:15am and 11:45am. There were 2 children present with 1 provider/vendor supervising. The physical plant, policies and procedures, emergency supplies and procedures, 3 children's records, 1 provider/vendor record and 1 household member records were reviewed. The exit interview was conducted with the provider/vendor. Areas of non-compliance are identified in this report. Violations:
Standard #: 8VAC20-790-190-A-18
Description: Based on review of children's records, there isn't a written agreements between the parent and the r in each record.
Evidence
Child #1 and #2 do not have a childcare agreement in their records.

Plan of Correction: We moved and I misplaced some documents. I do have everything and I will make sure all required documents are in each record.

Standard #: 8VAC20-790-190-B
Description: Based on review of children's record, all the records are not complete.
Evidence
The records for child #1, #2 and #3 do not include documentation of review of proof of identity, or permission for medical treatment.
The record for child #1 does not include parent name, address, phone number, place of employment, work phone number, emergency contact information, people authorized to pick up, child's name, and address.

Plan of Correction: I will get all missing information for the child's records: birth certificates, permission for medical treatment, authorized pick ups etc.

Standard #: 8VAC20-790-260-A
Description: Based on observation, areas of the family day home inside were not maintained in a safe condition.
Evidence
In the entrance hallway there were approximately 25 packages piled up on the floor. In the hallway outside the bathroom, there were approximately 12 boxes and packages piled up on the floor. These packages and boxes were accessible to the children causing a possible tripping hazard.

Plan of Correction: I keep clean my entrance hallway but that was unexpected. Entrance and hallway have all been cleaned up.

Standard #: 8VAC20-790-260-K
Description: Based on observation, stairs were accessible to children less than two years of age.
Evidence
The stairs going to the upstairs and to the downstairs did not have gates and were accessible to the children.

Plan of Correction: I have a gate for kids that goes in between the living room and stairs. I will make sure it is always up.

Standard #: 8VAC20-790-270-A
Description: Based on observation, potentially poisonous substances were accessible to the children.
Evidence
A spray (keep out of reach of children) was located in the bathroom and accessible to the children
Bleach wipes were on the kitchen island, making them accessible to the children.

Plan of Correction: I removed the spray and bleach wipes from the areas. I will not have anything accessible to the children.

Standard #: 8VAC20-790-280-A-4
Description: Based on observation the bathroom the children use was not equipped with paper towels.
Evidence
There were not paper towels available in the bathroom.

Plan of Correction: The bathroom now has the paper towels, we will make sure they are always there.

Standard #: 8VAC20-790-360-F
Description: Based on observation a crib was not provided for a child birth through 12 months of age for sleeping or resting.
Evidence
A child 11 months of age did not have a crib available for rest periods.

Plan of Correction: I have a crib, the child doesn't like to sleep in it. I will make sure all children under the age of 1 years old sleeps in a crib.

Standard #: 8VAC20-790-360-G
Description: Based on observation, rest mats were being occupied by more than one child at a time.
Evidence
During rest time, one rest mat is occupied by more than one child at a time, two children use the same mat.

Plan of Correction: One of the children doesn't like to sleep on the mat, she usually sleeps on the sofa. I did purchase another mat for the child.

Standard #: 8VAC20-790-420-C
Description: Based on observation, a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center was not posted in a visible and conspicuous place.
Evidence
There are not numbers posted for 911, emergency medical services or poison control center.

Plan of Correction: I will post 911 number, inspector will get me sticker or magnet with emergency numbers.

Standard #: 8VAC20-790-430-A-1
Description: Based on review of evacuation procedures, drills are not being practiced at least monthly.
Evidence
An evacuation drill was not practiced 10/22 through 4/23 and 8/23 through 8/23.

Plan of Correction: I misplaced the drills record, but since found it. I will continue to record all monthly drills.
