Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios (grouping of children), CPR, out of state searches, and accessing the department's website for memos, forms, and additional information.

Comments:
An unannounced monitoring inspection was conducted today, June 21, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 2:00pm and concluded at approximately 4:15pm. Today 25 children (ages 3-12yrs) were in attendance being supervised by four staff with the correct child/teacher ratio in each classroom. Administrative records, children's classrooms, restrooms, playground, and common areas were inspected. See the attached notice for the violation found during this inspection. Standard #: 22VAC40-191-40-D-2
Description: Based on the inspection of staff records, the required background check was not conducted as required.

Evidence:
During the inspection of five staff records, it was discovered that all five staff did not have a central registry background check in the record. Religious Exempt requirements stipulate all applicants for employment, employees, applicants to serve as volunteers, and volunteers and any other person who is expected to be alone with one or more children enrolled in the child day center are to obtain the required central registry background checks.

Plan of Correction: Per director: I will have staff to complete the form, then have it notorized before mailing them off.
