A Subsidy Health and Safety Inspection was conducted on 8/22/2023 from 11:54 am- 12:50pm. There were no children present during the inspection. The home, emergency supplies, and drills were inspected. Provider information and household member information was reviewed.

Standard #: 8VAC20-790-170-C Description: Based on record review and interview, the vendor did not obtain a sworn statement of affirmaiton for 1 household member.
Evidence:
1. The provider stated Household member #1 began residing in the home in June 2023. Household member #1 is over the age of 18.
2. The record for Household member #1 did not contain documentation of a sworn statement of affirmation.Plan of Correction: A sworn statement of affirmation will be completed.
Standard #: 8VAC20-790-250-D-1 Description: Based on record review, the provider did not have current certification in cardiopulmonary resuscitation (CPR).
