An unannounced Subsidy Health and Safety inspection was conducted today from approximately 11:15am-12:50pm. There was 1 child present directly supervised by the subsidy provider. The child was observed crawling and interacting with vendor. The home, emergency supplies and plans were inspected. Children?s records, Vendor records and household member documentation was also reviewed. There were no medications on site today. Areas of non-compliance are identified in the Violation Notice. Standard #: VENDHOM-000-005-B Description: Based on review of required documentation it was determined that a written hard copy record of daily attendance that documents the arrival and departure of each child in care was not maintained. Evidence: During inspection vendor did not have a daily record of the children in care for the day of the inspection or past days of operation. Plan of Correction: Ok, I will use the daily attendance sheet.
Standard #: VENDHOM-000-006-B Description: Based on review of child information it was determined that all required information was not maintained. Evidence During inspection Child1 and Child2 was missing: parents' work or school and phone number, emergency contact, authorized pickup, immunization, child care agreements, authorization for emergency care, documentation of special needs. and authorization for transportation. Child3 was missing: Home address, parents' work or school and phone number, emergency contact, authorized pickup, immunization, child care agreements, authorization for emergency care, documentation of special needs. and authorization for transportation. Child4 was missing: Date of birth, home address, parents' work or school and phone number, emergency contact, authorized pickup, immunization, child care agreements, authorization for emergency care, documentation of special needs. and authorization for transportation. Plan of Correction: I know the information, but will ask the parents to fill it out when they pick up the children.
Standard #: VENDHOM-000-023-F Description: Based on inspection of Family Day Home it was determined that a crib for children from birth through 12 months of age was not provided. Evidence: During inspection vendor did not have a crib available for a 10 month old child in care. Plan of Correction: I already ordered the crib it should coming soon.
