An unannounced monitoring Code compliance inspection was conducted today, 7/18/19. A violation notice was completed regarding the violations found during today's inspection. There are 57 children in care ranging in age from nine weeks to five years with 20 staff present. We discussed MAT training and field trips. Should you have any questions please do not hesitate to contact your inspector. Inspection began: 10:10 am and ended 1:30 pm.

Description: Based on review of staff records the center did not have all staff complete a search of central registry within 30 days of employment. Evidence: 1. Staff # 1 has a hire date of 8/19/18 and does not have a central registry (CPS) record check on file.

Plan of Correction: The director stated these have been sent off and was returned due to some errors in filling out the forms. The center is looking for this record.

Standard #: 63.2(17)-1716-A
Description: Based on observation the center did not have posted the fact that the center was exempt from licensure. Evidence: 1. The center has not posted in a visible location on the premises, the fact that the center is exempt from licensure.

Plan of Correction: The director stated that she will have this information posted in an area visible to the parents/guardians.
