A subsidy health and safety (SHSI) inspection was conducted today between 10:00 and 10:45am.. There were no children present with 1 caregiver/vendor. The physical plant, policies and procedures, emergency supplies and procedures, 1 provider/vendor record and 3 household member records were reviewed. The exit interview was conducted with the provider/vendor. Areas of non-compliance are identified in this report. Standard #: 8VAC20-790-170-C Description: Based on review of household member records, the caregiver did not ensure that the household member did undergo a background check.
Evidence:
The fourteen year-old household member did not have a Central Registry Search.Plan of Correction: The caregiver stated that the background check had been submitted and she is awaiting results.
Standard #: 8VAC20-790-200-2-f Description: Based on review of caregiver and household member records, there was no documentation of subsequent background checks conducted every five years.
Evidence:
The caregiver did not have documents showing a sworn statement and updated fingerprint check conducted every five years.Plan of Correction: The caregiver stated that all background checks had been submitted and she is awaiting results.
