A Subsidy Health and Safety Inspection was conducted on 1/18/2023 from 12:15pm through 1:05pm. There was 1 child directly supervised by the vendor. During the inspection the home, activities and interactions with children, emergency supplies, procedures and drills were inspected. Two children's records, vendor information and household member information was reviewed.
Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Evidence: Child #1s record does not contain the address of their emergency contact.Plan of Correction: I will have the parents give me the address today.
Standard #: 8VAC20-790-250-F Description: Based on interview, the vendor did not complete the required annual training.
Evidence: The vendor stated that they have not completed the departments annual health and safety update course.Plan of Correction: If you send me the link I can do it today.

Evidence: The vendor stated that they did not have an ice pack.Plan of Correction: I will send my husband to get one.
