An unannounced subsidy health and safety inspection (SHSI) was conducted at 11:40am. Observed 3 children = 8 points with the vendor. Ratios and supervision were incompliance. A household members were also present. The children were playing with napping during the inspection. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. An area of non-compliance with subsidy requirements and laws reviewed was found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 8VAC20-790-190-B Description: Based on records reviewed, 3 of 3 children's records did not include documentation of required information. Evidence: The records for Child #2, #3 and #4 did not include verification of the child's proof of identity. Plan of Correction: All child records will be corrected to show proof of identity was verified.
