An unannounced monitoring inspection was conducted on August 16, 2019 from approximately 12:20pm-2:15pm. Upon arrival to the facility, there were 43 children present with five staff members. Additional staff were on site for administrative and support purposes. The children were napping during the inspection. Four classrooms and two classrooms. The center does not offer transportation.

Standard #: 22VAC40-191-40-D-2 Description: Based on record review, in one of five staff records reviewed, the center did not ensure that staff have a Central Registry finding within 30 days of employment. Evidence: The Director confirmed that there was not a Central Registry finding for Staff #5 (date of hire 8/31/15) Plan of Correction: The staff member has completed the form and it will be mailed Friday.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, in three of five staff records reviewed, the center did not ensure that staff submit to fingerprinting and obtain results. Evidence: The Director confirmed that there were no fingerprint results for Staff #2 (date of hire 9/22/17), Staff #4 (date of hire 2015) and Staff #5 (date of hire 8/31/15). Plan of Correction: Anyone hired will have fingerprint results before they start.
