Comments:
An unannounced monitoring inspection was conducted on August 16, 2019 from approximately 2:20pm-5:00pm. Upon arrival to the facility there were 22 children with eight staff members. The children were observed were playing freely. The center does not offer transportation. Eight rooms were reviewed and the outdoor playground areas. Five children's records and five staff records were reviewed.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review, in five of five staff records reviewed, the center did not ensure that all staff have a completed sworn statement prior to employment and a Central Registry finding within 30 days of employment. Evidence: The Director confirmed that the following staff members did not have a completed sworn statement or a Central Registry finding: Staff #1-date of hire 5/6/19 Staff #2-date of hire 3/6/19 Staff #3-date of hire 1/27/17 Staff #4-date of hire 9/7/17 Staff #5-date of hire 8/25/16

Plan of Correction: Teachers return on August 26th. Every document will be completed and sent to the licensing inspector.

Standard #: 63.2(17)-1716-A-4
Description: Based on record review, in one of five staff records reviewed, the center did not ensure that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: The Assistant Director confirmed that there was no documentation of a staff health report for Staff #2 (date of hire 3/6/19).

Plan of Correction: The staff member will bring in the staff health report on August 26th. Going forward, we will make sure that we have that before they begin.

Standard #: 63.2(17)-1716-B-7
Description: Based on inspection of the facility, the center did not ensure to establish and implement procedures for reports of serious injury or death of children attending the child day center. Evidence: The Director confirmed that the center had not established and implemented the following procedures: 1. All incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner. 2. Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually. 3. Reports of deaths shall be submitted no later than one business day after the death occurred.

Plan of Correction: All procedures and policies will be updated in our handbook. We will go over the information with our staff during orientation.
