Technical Assistance:
Consultation was given on maintaining all files with subsidy requirements, training opportunities on-line, model forms for subsidy (Fairfax County) as well as reviewing all vendor requirements for the home.

Background Checks are processed by the City of Alexandria.

Comments:
An in-person annual subsidy inspection was conducted on 3/7/2024 from 11:20-12:15. There was one child present with the vendor. The inspector reviewed 2 children's records and adult/household member background checks and TB clearance documentation, physical space, trainings and nutrition. Information gathered during the inspection determined areas of non-compliance with applicable regulations or laws and violations were documented on the violation notice issued to the vendor. Description: Based on vendor statement and Child Care worker, the criminal record check for the vendor has not been repeated within 5 years.
Evidence"
The Vendor's Criminal Record Check expired in December 2023.

Plan of Correction: The Vendor has had the fingerprints done on 2/24/2024 and is waiting for the results.

Description: Based on record review, a signed permission to transport a child was not in the file.

The vendor picks up Child #1 from school every day and no permission is available.

Description: Based on vendor statement, no written emergency plan has been created to address emergency situations and responses.
Evidence:
The vendor stated that she did not have a written plan.

Plan of Correction: The model from Fairfax County was emailed to the vendor for her to complete the plan and keep with emergency drills/log.
