Comments:
An unannounced monitoring inspection was conducted on August 28, 2019 from approximately 9:30am-12:00pm. There were no children present but two staff members were present. Two classrooms, two restroom areas and the outdoor playground were inspected. The center does not offer transportation. The center has a current enrollment of 30 children. Five children's records and five staff records were reviewed.

Violations:
Standard #: 63.2(17)-1716-B-7
Description: Based on observation and inspection of the facility, the center did not establish and implement procedures to ensure that all requirements are followed with regards to reports of serious injury or death of children while attending the child day center.

Evidence: The Director confirmed that the center had not established the following procedures:
1. Ensuring that all incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner.
2. Reports of serious physical injuries shall be submitted annually.
3. Reports of deaths shall be submitted no later than one business day after the death occurred.

Plan of Correction: Director will create those procedures and meet with the staff to go over those procedures.

Description: Based on record review, in two of five staff records reviewed, the center did not ensure that staff have a five year repeat of required background checks.

Evidence: The Director confirmed that the following background checks for staff members were more than five years old:
Staff #1 (date of hire 6/17/11)-Central Registry results dated 6/27/11 and Sworn Statement dated 6/17/11

Staff #2 (date of hire 8/13/12)-Sworn Statement dated 8/13/12

Plan of Correction: Director will contact the staff members and get the paperwork filled out and mailed. The results will be forwarded to the licensing inspector.
