Standard #: 63.2(17)-1716-B-4 Description: Based on record review and interview, the center failed to implement a procedure to ensure that all children in the center are in compliance with the immunization provisions.
1. The record for child 4, date of birth 3/14/17, contains an immunization record dated 3/5/18.
2. The director verified the 3/5/18 immunization record is the most current record they have. The center policy is for immunizations to be obtained prior to the first day of care and for children under the age of two every six months.
Plan of Correction: An updated immunization record will be obtained for child 4. In the future the administration will ensure all immunization records are up-to-date.
