An unannounced Code inspection was completed today, 9/18/19.
Upon arrival to the center the classes were busy with picture day. Lunch and hand washing was also observed. We discussed the Code requirements and a review of staff and children's records was also conducted. Playground was also observed.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of staff records the center failed to have all staff submit the required background checks information.
Evidence:
1. Staff #1-7 do not have a central registry record check on file. All staff have been employed more than 30 days.
2. Staff #1-7 do not have a sworn statement or affirmation before employment or commencement of service at the center.Plan of Correction: The director will ensure that all staff complete the required central registry requests and the sworn affirmation forms.
Standard #: 63.2(17)-1716-A Description: Based on review of records and interviews the center failed to have all the required information posted and disclosed to the parents/guardians.
Evidence:
1. The fact that the center is exempt from licensure is not posted in an area that is visible.
2. The written information provided to the parents does not include information that the center is exempt from licensure.Plan of Correction: The director will ensure that all this information is provided in writing to parents/guardians.
