Comments:
An unannounced monitoring inspection was conducted at First United Methodist Church on 10/7/19 from 10:00 am until 12:00 pm to review the religious exempt requirements. At the time of the inspection 26 children were present with five staff. The sample size consisted of five children's records and six staff's records. Children and staff were observed during indoor free play, an educational lesson, bathroom break, hand washing, reading time, clean-up, transitions and behavioral guidance. Description: Based on record review and interview, the center failed to establish and implement a procedure in which a daily health screening and exclusion of sick children by a person trained to perform such a screening is conducted daily.

Evidence:

1. The records of six staff were reviewed. No staff record contained documentation of completing daily health training.
2. The director stated no staff have been formally trained in daily health screening. The center does have a sick child exclusion policy and the staff do observe children daily for signs of illness.

Plan of Correction: The staff that observe children upon arrival will be trained in daily health screening.

Description: Based on record review and interview, the center failed to obtain a search of child abuse and neglect registry or equivalent registry maintained by any other state in which a new staff has resided in the preceding five years for any founded complaint of child abuse or neglect against him/her by the end of the 30th day of employment.

Evidence:

1. The records of six staff were reviewed. The record of staff 6 has documentation of living in CA within the last five years and staff 6's start date is 8/19. There was no search of child abuse and neglect registry or equivalent registry from CA in staff 6's record.
2. The director stated they have not completed a search of child abuse and neglect registry or equivalent registry from CA.

Plan of Correction: A search of child abuse and neglect registry or equivalent registry for CA will be requested for staff 6. In the future any new staff that are hired and have documented on the sworn statement they have lived outside of VA in the last five years will have a request for a search of child abuse and neglect registry or equivalent registry done in the state they resided in sent within 30 days after employment.
