An unannounced complaint inspection was conducted on 10/3/19 from 11:15 to 12:35 in response to a complaint that was received by the licensing office on 9/12/2019 regarding alleged noncompliance in the areas of physical plant maintenance, staff to child ratios, supervision, emergency preparedness and discipline.
The information gathered during the investigation supports the allegation of physical plant maintenance of the outdoor play equipment. This portion of the complaint was Valid and the violation appears on the violation notice.
The information gathered during the investigation does not support the allegations regarding to staff to child ratios, supervision, emergency preparedness and discipline so this portion of the complaint is found to be Not Valid.

Standard #: 63.2(17)-1716-B-5 Complaint related: No Description: Based on observation and interview, the facility did not ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
Evidence:
The inspector observed a link on the chain to one swing on the outdoor play space that had rusted completely through and was no longer an enclosed loop. There were other minor areas of rust on the swing chains which did not appear to have deteriorated the chain, but that should be very carefully monitored and replaced s needed.Plan of Correction: The program director removed the swing during the inspection and stated that she will closely monitor the condition of the outdoor play equipment to ensure they are not compromised.
