9:40-11:00am Unannounced religious exempt inspection conducted for the playschool program. Observed 6 children + 2 direct-care staff. Ratios and supervision were in compliance. Children were observed playing with toys. The facility was found to be clean, safeguarded appropriately, and well supplied with toys and equipment for the children. An outdoor playground allows for active play. Administrative and safety non-compliances found.

Standard #: 22VAC40-191-40-D-2 Description: Based on records reviewed, it was determined that 3 of 3 staff records did not include required background checks.
Evidence:
1. Staff #1 - Sworn Disclosure Statement (SDS), and Central Registry Check (CPS). Hired August 2019.
2. Staff #2 - SDS, and CPS. Hired 2014.
3. Staff #3 - SDS, and CPS. Hired 2015.Plan of Correction: Each will complete a SDS in the next 10-days. Each will submit a CPS in the next 10-days. Copies of these documents will be sent to the inspector in the next 10-days.
SDS completed 11/6/19.
CPS to be mailed by end of week.
