Licensing Inspector arrived at 9:45 and departed at 11:00 AM to conduct a religiously exempt child day center code compliance inspection. At the time of the inspection, there were 25 children (ages three and four years) in care with six staff. Children were observed doing art activities, playing with toys and doing a letter activity. The sample size was six staff.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of six staff records, it was determined that the facility did not ensure that all staff had a completed sworn statement or affirmation completed prior to employment.
Evidence:
The record for staff #6 did not contain a completed sworn statement or affirmation.Plan of Correction: Center management will have staff member complete sworn statement and place it in record. In the future, all staff will complete these prior to employment.
Standard #: 63.2(17)-1720.1-A Description: Based on review of six staff records, it was determined that the facility did not ensure that all background checks were updated every five years.
Evidence:
The following staff had search of the central registry checks (CPS) that were over five years old:
- Staff #2 (last check dated 9/11/14)
- Staff #3 (last check dated 11/30/11)Plan of Correction: Center management will ensure that all checks are updated and results placed in staff records.
