Licensing Inspector arrived at 9:45 and departed at 11:00 AM to conduct a religiously exempt code compliance inspection. At the time of the inspection, there were 33 preschool aged children present with four staff. This facility also operates a private school. Children were observed participating in circle time activities. The sample size was 11 staff records.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of 11 staff records, it was determined that the facility did not ensure that a completed search of the central registry was on file for each staff member with 30 days of employment.
Evidence:
The record for staff #11 did not contain a completed search of the central registry.Plan of Correction: Center management will ensure that all staff have completed search of the central registry on file.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of 11 staff records, it was determined that all staff have completed fingerprints on file prior to employment.
Evidence:
The following staff records did not contain fingerprints as required by the Code of Virginia: staff #1, staff #2, staff #3, staff #4, staff #5, staff #6, staff #8, staff #9, staff #11.Plan of Correction: Fingerprints have already been received for staff #5, staff #8 and staff #9. All other staff will obtain fingerprints as required. In the future all staff will have fingerprints prior to employment
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of 11 staff records, it was determined that the facility did not obtain out-of-state search of the central registry for staff who indicate that they have lived out of state in the past five years.
Evidence:
The following staff, who indicated that they had lived out of state in the last five years, did not have search of the central registry for those states: staff #3 (Delaware), staff #6 (Iowa)Plan of Correction: Out of state checks have been submitted. Once received results will be placed in records.
