Comments:
A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on January 30, 2020, from 1:40 p.m. to 3:25 p.m. At the time of arrival the following staff to child ratios were observed:
10 Children, 4 years old, with 1 staff
4 Children, 4 years old, with 1 staff
9 Children, 4 to 5 years old, with 1 staff
10 Children, 3 to 4 years old, with 1 staff
7 Children, 4 to 5 years old, with 1 staff
7 Children, 4 years old, with 1 staff
8 Children, 5 to 6 years old, with 1 staff
7 Children, 5 to 6 years old, with 1 staff
6 Children, 5 years old, with 1 staff
8 Children, 2 ? to 3 years old, with 1 staff
Children were observed resting quietly at nap time, engaging in small group learning centers, being read to and having an afternoon snack.
Sixteen staff records were reviewed, posted information and parent handbooks were reviewed and the center program?s administrative team was interviewed regarding the centers policies and procedures. The facility, to include all indoor areas used by preschool children and before and after care children, as well as the outdoor play space, were observed.
Staff report that the program does not routinely transport preschool children in a vehicle.

Standard #: 22VAC40-191-40-D-2
Description: Based on a review of records and interview, the Religious Exempt center did not ensure that the results of a Search of the Central Registry are obtained by the center within 30 days of employment of staff.
Evidence:
Sixteen staff records were reviewed.
The record for staff #10 whose employment date is documented as 8/23/19 did not contain the results of a Search of the Central Registry.
The Program Administrator felt sure that these results had been obtained but was unable to locate the documentation.
The program administrator was unable to obtain documentation of these results.

Plan of Correction: The program administrator stated that she will contact the agency to see if documentation of the background check is available. If this documentation is not available, then a new request for a search of the central registry will be sent out by February 13, 2020.
