Comments:
An unannounced monitoring inspection was conducted on 02/10/2020. There were 41 children, ages 3 months-4 years, and 8 staff members present during the inspection. The inspector reviewed staff information for 9 staff members during the inspection. The children were observed in the following activities: in free choice activities, during lunch and going down for a nap. The inspector discussed best practices in the areas of policies/procedures, attendance tracking, record keeping, training and supervision with the program director. Two inspectors arrived for the inspection at 10:00 a.m. and departed at 2:00.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 63.2(17)-1716-A
Description: Based on observations and discussion with the staff, the facility failed to ensure that the exempt from licensure is posted in a visible location on the premises.

Evidence:The letter of exemption was not posted in Building #1 (the infant-2 year old building).

Plan of Correction: A copy will be made and posted in Building #1.

Description: Based on document review and discussion with the staff, the facility failed to ensure that staff who transport children must have a valid Virginia driver license or commercial driver's license and that the vehicles used to transport children must be insured.

Evidence:
1. Staff #1 stated that there are 6 staff members that are able to drive facility vehicles to transport children. Staff #1 could only provide documentation to verify that 3 of the 6 vehicle drivers held a Virginia driver's license or commercial drivers license.
2. The insurance documentation that was provided expired 04/2019; up-to-date documentation could not be provided during the inspection.

Plan of Correction: Copies of all driver's licenses and the Insurance verification will be obtained.

Standard #: 63.2(17)-1716-B-5
Description: Based on observations and discussions with staff, the facility failed to ensure that all areas of the premises accessible to children are free of obvious injury hazards.

Evidence:
1. There were multiple uncovered electrical outlets that were accessible to children in both Building #1 and Building #2. Children were observed playing in and around areas that had accessible uncovered electrical outlets during the inspection.
2. There was a mop bucket that contained water that was accessible to children in one of the infant areas of Building #1.
3. There were 2 bottles of bleach on the floor that were accessible to children.

Plan of Correction: The bleach and mop bucket will be moved to the kitchen area. The outlets will be covered.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and discussion with the staff, the facility failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.

Evidence: Staff #9 has a documented hire date of 04/29/2019; the date on the fingerprint eligibility letter is 05/07/2019.

Plan of Correction: Staff will not be hired until the eligibility letter is received.
