An unannounced monitoring inspection was conducted on 2/10/20 from 10:00 am until 1:30 pm at Harrisonburg First Church of the Nazarene to review the religious exempt requirements. At the time of the inspection 96 children were present with 20 staff. The sample size consisted of nine children's records and nine staff's records. Children and staff were observed during free indoor play, hand washing, bathroom break, diapering, educational and artistic activities, story time, lunch, bottle feeding, departure, transitions, table cleaning and sanitizing and behavioral management. Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and interview, the center failed to obtain an out-of-state child abuse and neglect search for staff who have resided outside of VA within the past five years of the day of employment by the end of the 30th day of employment.
Evidence:
1. The records of nine staff were reviewed. Staff 5's start date was documented as 10/1/19. The sworn statement documented staff 5 lived in NC within the last five years. There was no search of child abuse and neglect completed for NC in staff 5's record.
2. The director and office worker were unable to locate a search of child abuse and neglect for NC for staff 5.Plan of Correction: The paperwork was printed and completed during the inspection. Staff 5 coming today to sign the paperwork and get it notarized. The document will be mailed tomorrow.
