Comments:
An unannounced monitoring inspection was conducted on February 12, 2020 from approximately 9:35 am-12:20 pm. Upon arrival to the facility, there were 71 children present with nine staff members. Additional staff were on site for administrative and support purposes. Seven staff records were reviewed along with the center's required written disclosures to parents, guardians and the general public. The children were observed in nine classrooms doing a variety of activities to include playing in centers, working on Valentine's Day crafts, journaling and having assessments done by the teachers. The outdoor play area was inspected. The center does not offer transportation.

Violations:
Standard #: 63.2(17)-1716-B-7
Description: Based on staff interview, the center did not establish and implement procedures regarding reports of serious injury or death of children while attending the center.

Evidence: The Director of Mother's Day Out confirmed that the center had not established and implemented the following procedures:

1. Ensuring that all incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner.

2. Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually.

3. Reports of deaths shall be submitted no later than one business day after the death occurred.

Plan of Correction: We will follow the procedures and implement them so that employees know the correct procedures regarding notification to the state and the Commissioner so the Director can instruct the staff of the preferred notification through the public website.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, in two of seven staff records reviewed, the center did not ensure that staff submit to fingerprinting and obtain results prior to employment.

Evidence: The Mother's Day Out Director confirmed that there were no fingerprint results lettersavailable for review for the following staff members: Staff # 4 (date of hire 3/2019) and Staff #7 (date of hire 2011).

Plan of Correction: Staff #4 will be re-fingerprinted because the paperwork cannot be found. We will contact the state on further guidance for obtaining the results for Staff #7.
