Comments:
A compliance inspection to monitor the requirements for Religiously Exempt Childcare Centers was conducted onJanuary 30, 2020, from 8:15 a.m. to 10:45 a.m. At the time of arrival the following staff to child ratios were observed:
4 infants with 1 staff
10 toddlers (16 to 24 months old, with 2 staff
11 Children, 2 years old, with 1 staff
9 Children, 3 to 4 years old, with 1 staff
Children were observed eating breakfast, playing in learning centers, and being read to.
No new staff were noted since the last inspection and records were reviewed, as needed, for updates. Posted information and parent handbooks were reviewed and the center program?s administrative team was interviewed regarding the centers policies and procedures. The facility, to include all indoor areas used by preschool children and before and after care children, as well as the outdoor play space, were observed.
Staff report that the program does not routinely transport preschool children in a vehicle.
Repeat violations were noted in the areas of equipment / physical plant and appear on the violation notice

Description: Based on observation and interview, the facility did not ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.

The outdoor play space contained a variety of pieces of composite play equipment to include 8 climbers and slides as well as 2 sets of swings (8 and 6 swings each). None of these pieces of play equipment had cushioning material under and around them.
A child falling or jumping from any of the outdoor play equipment could sustain an injury.

Plan of Correction: The program administrator stated that she intends to decide upon a cushioning material and will install it under each piece of equipment within 30 days.
