An unannounced monitoring inspection was conducted on 3/9/20 from 11:20 am until 1:40 pm at Park Street Christian Church to review the religious exempt requirements. At the time of the inspection 21 children were present with four staff. The sample size consisted of five children's records and five staff's records. Children and staff were observed during free indoor and outdoor play, educational and artistic activities, lunch, hand washing, bathroom break, pick-up, parent communication, transition and behavioral guidance. Standard #: 63.2(17)-1720.1-A Description: Based on record review and interview, the center failed to have staff update the signed sworn disclosure statement every five years.
Evidence:
1. The records for five staff were reviewed. The most current sworn disclosure statement in staff 1's record was dated 9/19/14.
2. The director stated she thought they updated staff 1's sworn disclosure statement but could not find it.Plan of Correction: Staff 1's sworn disclosure statement was updated the same day as the inspection. In the future the director will ensure all background checks and sworn disclosure statements are updated every five years.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and interview, the center failed to obtain a search of child abuse and neglect registry or equivalent registry maintained by any other state in which a new staff has resided in the preceding five years for any founded complaint of child abuse or neglect against him/her by the end of the 30th day of employment.
Evidence:
1. The records for five staff were reviewed. Staff 3's start date was documented as 9/3/19. The sworn statement states staff 3 resided in TX, AZ, UT & WI within the last five years. There was no documentation of a search of child abuse and neglect registry being completed for each state.
2. The director verified the center failed to obtain a search of child abuse and neglect registry.Plan of Correction: During the inspection the director started to work on completing the a search of child abuse and neglect registry for each state. In the future the director will review the sworn statement upon hire and complete a search of child abuse and neglect registry for all new staff upon hire.
