This annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic. Preliminary data was obtained from the vendor on October 28, 2020. An annual subsidy inspection was initiated on November 2, 2020 which time the vendor was contacted by telephone to initiate the inspection, and concluded on November 4, 2020. The inspector emailed the vendor a list of items required to complete the inspection. The vendor reported there were 44 children in care. The inspector reviewed three children's records and three staff records submitted by the vendor to ensure compliance with the health and safety regulations. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Standard #: 22VAC40-665-520-B-3 Description: Based on record review, in three of three children's records reviewed, the vendor did not ensure that each child's record included all required information.
Evidence: The records for Child #1, Child #2 and Child #3 did not include the work addresses for one or both parents.Plan of Correction: I will review the folders again and contact the parents. If there is anything missing I will ask the parents to update them.
