Comments:
This inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic. The department received a complaint regarding special care provisions and emergencies, and a subsequent investigation was initiated on December 28, 2020 and concluded on January 08, 2021. The inspector contacted the vendor by telephone to conduct the investigation. The inspector had a conversation with the vendor regarding a list of documentation required to complete the investigation, interviewed additional staff and viewed documents provided by the vendor. The evidence gathered during the investigation supported the allegations of non-compliance with standards or law, and a violation was issued. Standard #: 22VAC40-665-550-2
Description: Based on staff interviews and a review of documentation provided by the center, the vendor did not ensure to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.

Evidence: The director and staff interviews confirmed that on November 13, 2020, Child #1 (two years old) fell at the center and sustained an injury that was allegedly unbeknownst to the staff at that time. After being made aware that the child was injured and taken to a medical center that evening, the vendor did not inform the department's representative.

Plan of Correction: The center will ensure that if a child sustains an injury while in our care at our center and has to seek medical attention because of it, we will file the Serious Injury Report Online within 2 business days.
