Technical Assistance:
Social security cards are not acceptable forms of proof of identity. The following are acceptable forms of proof of identity: certified copy of a birth certificate, birth registration card, notification of birth (hospital, physician, or midwife record), passport, copy of placement agreement or entrustment agreement from a child placing agency (foster care and adoption agencies), record from a public school in VA), certification by a principal or his designee of a public school in the US that a certified copy of the child's birth record was previously presented, copy of the conferring temporary legal custody or entrustment agreement of a child to an independent foster parent and child identification card issued by the VA DMV.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
An unannounced code compliance inspection was initiated on 1/31/2022 and concluded on 2/1/2022. The person in-charge was contacted by telephone and a virtual inspection was conducted. There were 55 children present, ranging in ages from two to five, with 13 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of five children?s records and five staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review and interview, the center failed to obtain out of state sex offender registry search for each state a staff member resided in the past five years prior to the first day of employment.

The sworn statement for staff 2 documents living in Ohio and California within the past five years prior to the first day of employment. The staff record does not have documentation of completing sex offender registry searches in Ohio and California.

Plan of Correction: Sex offender registry searches will be completed for staff 3. In the future the administration will ensure out of state sex offender searches will be completed as required.
